  3. Kindly confirm your Ghana Card PIN
  4. Enter your Surname on Ghana Card
  5. Enter your First Name(s) on Ghana Card
  6. Enter your Date of Birth (Sample: DDMMYYYY)
  7. Select Gender
  8. Confirm your Personal details submitted
  9. Click on the submit details after confirmation
